Money Follows the Person: A Program for Nursing Home Residents Who are Interested in Transitioning Home

Funded through a federal grant from the Centers for Medicare and Medicaid Services, Money Follows the Person is a program designed to transition those who reside in nursing facilities, and other institutional settings, to their homes or other community-based settings.

To be eligible for this program, the following requirements must be met by the resident:

  • Must be a current resident of a qualifying institution (nursing facility)
  • Must have been a resident for the past 90 consecutive days (not counting Medicare days)
  • Must have needs that can be safely met through services in the community
  • Must be an Indiana Medicaid-eligible resident at least one day prior to discharge from the institution
  • Must meet the minimum requirements for a funding source that is currently partnering with Indiana’s Money Follows the Person program

Once these requirements are met, the Money Follows the Person program can potentially provide case management services to assess needs and assist in setting up housing, medical equipment and in-home services that will be needed in the home setting. The program also provides ongoing case management to monitor their safety and services to ensure that all needs are being met.

  • Home-delivered meals
  • Personal Emergency Response Systems
  • Attendant care
  • Adult day care
  • Medication administration systems
  • Structured family caregiving
  • Home and environmental modifications
  • Respite RN
  • Pest control
